Addressing Social Needs to Promote Health in Ohio

Although the powerful influence of social circumstances and individual social needs on health and disease outcomes are well documented, clinical care systems in Ohio face ongoing challenges in addressing patients’ social needs. Health care organizations have struggled with collecting data on social needs, how to prioritize specific social needs over others, and integrating referrals and social services into clinical care environments. To help clinics and providers address these challenges, these case studies highlight efforts that Ohio health care organizations are engaging in to promote health by addressing the social needs of Medicaid populations in partnership with the community.
