About the Ohio Cardiovascular and Diabetes Health Collaborative (Cardi-OH)
Cardi-OH is a statewide network of seven academic medical institutions funded by the Ohio Department of Medicaid. Cardi-OH is focused on improving cardiovascular and diabetes health outcomes and eliminating disparities in Ohio's Medicaid population.
About Project ECHO®
Project ECHO® (Extension for Community Healthcare Outcomes) is an innovative educational program developed at the University of New Mexico Health Sciences Center. Cardi-OH has partnered with Project ECHO® to increase the capacity of primary care providers to safely and effectively treat cardiovascular disease.
Cardi-OH ECHO uses a combination of multi-point videoconferencing technology, casebased learning, promotion of best practices, and the monitoring of outcomes to create and support professional communities of learning and practice.

Structure of TeleECHO Clinics

- Apply recommended strategies to communicate weight-related issues to a diverse range of patients
- Explain key determinants of health behavior change related to weight
- Identify and address social determinants of health and mental health barriers to achieving and maintaining a healthy weight
- Demonstrate cultural competence in addressing weight management among diverse populations
CME Accreditation/Designation Statement
The MetroHealth System is accredited by the Ohio State Medical Association to provide Continuing Medical Education (CME) for physicians.
The MetroHealth System designates this educational activity for a maximum of 12 AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity.
Other Healthcare Professionals: check with your professional association as these credits might be applicable for licensure renewal.
Expectations of Participation
By registering, you confirm your acknowledgement and consent to participate in the Cardi-OH ECHO clinic, and agree to:
Participate collegially in regularly scheduled Cardi-OH ECHO sessions by presenting cases, providing comments, asking questions;
Keep confidential any patient information provided by other participants during a session;
Complete periodic surveys to help improve services to clinicians and other partners;
Use required software including, but not limited to Zoom and Box;
Provide clinical updates and de-identified outcome data on patients as needed;
Be solely responsible for the treatment of your patients and understand that all clinical decisions rest with you regardless of recommendations provided by other Cardi-OH ECHO participants and;
Ensure that your patients are aware of your participation in Cardi-OH ECHO and their de-identified information could be shared.
You will present one to two case studies during the 12-week series (you will be scheduled to present on a specific date).
The submission of cases for presentation and discussion is a key component in the Project ECHO® model and critically important for knowledge building and sharing. We welcome cases that involve common clinical scenarios as well as difficult, complex, or challenging presentations.
A Case Study Form will be provided two weeks prior to your case presentation date to collect demographic and relevant clinical information about the patient. Please submit the form one week prior to the presentation date. This is an electronic form that will be assigned an ECHO ID # upon submission and returned to you as a PDF copy for your files prior to your case presentation date.
NO HIPAA IDENTIFIERS MAY BE MENTIONED OR SHOWN DURING CASE PRESENTATIONS. In addition, no other information that might identify the patient - such as social history details that may identify a patient residing in a small community - may be mentioned.
During the Cardi-OH ECHO clinic, the Facilitator will call on you to present your case. The Clinic Coordinator will scroll through your Case Study Form on screen as you present your case. Please verbally summarize your case in 10 minutes or less.
Please note that the Cardi-OH ECHO clinic will be recorded for educational purposes. By participating in a clinic, you are consenting to being recorded and photographed. Recordings will be made available on a secure website for access by Cardi-OH ECHO participants.
Complete the Pre-clinic and Needs Assessment Survey before the series begins (deadline to be determined).
Complete the Practice Profile (1 per practice) before the series begins (deadline to be determined).
Complete all Post-Clinic Surveys (each week during the last 5 minutes of each clinic).
Complete the Exit Survey after the series ends (deadline to be determined).
Use of Zoom Technology
Cardi-OH ECHO will support practices’ use of web conference technology (i.e., webcam, microphone/telephone audio, Zoom software) including a 1-on-1 tech check prior to the beginning of the 12-week series.
Participants are encouraged to be visible on video while in the clinic and contribute actively to the case discussion.
Cardi-OH ECHO will consider individuals interested in attending a clinic as an “Observer.” These individuals must submit a written request to the Clinic Coordinator and be granted permission to join. Observers will attend as non-participants and will be listen-only.
