Summary

Listen to Mamle Anim, MD, share lessons learned from Five Rivers Health Centers, a Federally Qualified Health Center in Dayton, Ohio with demonstrated success implementing quality improvement initiatives. Five Rivers Medical Surgical Health Center participated in the MEDTAPP Hypertension Quality Improvement Project and demonstrated improved blood pressure control for its African American patients. This is the second in a series of quality improvement education podcasts designed for primary care practice teams.
